Myocardial rupture associated with bolus injection of contrast medium during computed tomographic study in a patient with acute myocardial infarction: a rare but lethal complication
ثبت نشده
چکیده
Contrast-enhanced computed tomographic (CT) scans are being performed daily for many reasons. They are especially useful for the evaluation of patients with acute chest symptoms because myocardial infarction, pulmonary embolism, and aortic dissection have overlapping clinical presentations. Indeed, advances in technology and the introduction of multidetector CT scan have enabled better evaluation of the heart. Yet, the potential risks to the cardiovascular system of using a bolus injection of contrast medium, although rare, should not be ignored or underrated. Here, we report a rare case of complicated left ventricular free wall rupture after a single bolus injection of contrast medium during a CT study in an elderly woman with acute myocardial infarction. Such a case has not been reported in the literature to date. This should alert us to the potential, if rare, complications that use of a bolus injection of contrast medium may induce, and lead to more cautious use of contrast medium in the future.

Myocardial rupture associated with bolus injection of contrast medium during computed tomographic study in a patient with acute myocardial infarction: a rare but lethal complication.
Well-documented potential cardiovascular complications associated with the use of contrast media include bradycardia, hypotension, arrhythmia, and conduction disturbances. Rupture of the myocardium after acute myocardial infarction is a known cause of death, but has yet to be recognised as a potential complication of the use of a bolus injection of contrast medium. On the contrary, contrast-enh...
متن کاملManagement of a large coronary artery aneurysm with graft-coated stent during the acute phase of myocardial infarction
Coronary aneurysms in adults are rare clinical entities. Herein, we presented a 54-year-old man who was admitted with an acute extensive anterior myocardial infarction. The emergency coronary angiography revealed an isolated large aneurysm at the proximal segment of the left anterior descending coronary artery. The patient was successfully treated with a graft-coated stent

A rare complication of AMI is papillary muscle rupture (PMR), occurring in 1% of patients with acute myocardial infarction (AMI). Mitral regurgitation related to PMR occurs at a median of 1 day (ranging from 1-14 days) after onset of myocardial infarction. PMR tends to occur in those having a first time AMI and those without extensive collateral circulation. PMR involves posteromedial papillary...
متن کاملA Rare Case of Acute Myocardial Infarction Associated with Arsenic Poisoning
Background: Arsenic toxicity as a result of consumption of hair removal agent has been rarely seen in last few years in comparison with the past. Case Presentation: In this case report, we presented a middle age woman referred to medical toxicology department due to epigastric pain and diarrhea following ingestion of hair removal agent. Urinary test for arsenic was positive. ECG changes and ele...
متن کاملFactors associated with prehospital delay in patients with acute myocardial infarction
Introduction: Treatment of patients with acute myocardial infarction (AMI) is time related, so delay in treatment could affect prognosis. Recognizing pre-hospital or in-hospital delays in initiating treatment and reducing these factors is very efficacious in treatment of these patients. Methods: A cross sectional study was carried out on 227 patients with acute myocardial infarction. Demographi...
